Menstrual flow is the uterus lining shedding, so it originates in the uterus and would leave the uterus via the cervix. The cervix is at the top of the vaginal canal, the vaginal canal then is the passageway through which the menstrual flow passes.

The opening at the lower end of the uterus is the internal cervical os. It leads to the cervical canal, through the exernal os, and into the vagina.
The uterus is the organ behind the cervix in a female. It has nothing to do with penis size, as the penis never enters the uterus. The penis enters the vaginal canal, which is very elastic. The cervix is at the end of the vaginal canal. The only time the cervix opens enough to allow entry, is when a woman is birthing a child.
