Sounds like you're in medical school which means you should know this or be able to look it up rather than asking on here. After transplanting the lungs the medical problems could be infection or rejection.

when you breathe in the smoke in it, it goes into your lungs and your lungs get black and you have many problems other than problems with your lungs. so make sure you don't smoke! its bad for you and other people around you!
Most lung problems originate within the lungs, from a variety of causes. The only way the kidneys could contribute to or cause a lung problem is if the kidneys do not clear wastes and fluids, causing systemic fluid overload. For example, ascites, fluid in the abdomen, could cause pressure on the lungs and prevent the lungs from expanding fully. Severe kidney disease could lead to or be in conjunction with Congestive Heart Failure (CHF) which causes a backlog of fluid/blood, which can affect the lungs. Your lungs are the reasons you body has Oxygen. In your lungs, we do produce mucus, which we cough up because of the cilia in our lungs. Smoking causes this cilia to not move the mucus out of your lungs which in turn causes problems.
