To try and get rid of the Buboes they decided to use a thing called 'lancing'. This is when someone gets something sharp like a knife for eg. and they would try their best to slice through the Buboes until they burst. This was a struggle because the buboes were rock solid so it took a while for this to do so.

With boils and cysts when would surgery be required?

Considering that lancing a boil is technically surgery, I'd say any time the treatment proceeds below the derma. Note that, with boils at least, you're likely releasing an infective substance, so the path that substance takes upon release -- out of the body and not further in -- is critical. Any time that the problem of post-treatment sepsis cannot be assured without invasive measures, surgery is required. You should understand that when it becomes painful enough, you should go for the surgery to get the relief from this pain.

Why is it called the black plauge?

Because the bubonic plague (first to hit Britain) was when humans got boils, and the boils were BLACK. Because the Bubonic plague, (spread by fleas from infected rats), would cause the victim to receive boils, and blotches of skin that would turn black or blue. These are not boils but enlarged lymph nodes which became black (gangrenous).
