Its caused mainly by occlusion in the artery by an atherosclerotic plaque(imagine it as an obstacle which prevents sufficient blood to reach the area which needs blood).. So when this obstacle is big enough to reduce progressivly oxygen supply to that area of the heart(because this plaque gets bigger by time and when no therapy and managment is done).. Muscle dies and infarction happens.( shortly because of the death of myocytes)
