When the diaphragm contracts, negative pressure is created in the lungs. This is when people breath in, and obviously, breathing rates are signifigantly higher when a person runs. If the muscle is weak or unaccustomed to exercise at the present intensity, a spasm could result, causing that side stitch that all runners know too well. The liver may also be what causes the pain. It is the heaviest organ in the body, and the jarring effects of running will cause a bit of internal strain and discomfort. This also explains why running on a full stomach feels a lot worse than running on a mostly empty stomach.
