There are many reasons- Poor circulation, electrolyte imbalance, nerve injury..the list goes on. If it persists, the best course of action would be to see a doctor.

it depends how you train them. you could have a huge, hyper English sheperd and train it to be an inside dog. But they should be able to go outside whenever they need to.
