First question is why would you repeated kiss a toddler on the mouth? I think regardless of whether it agitates the toddler's cold sore, you should stop kissing it on the mouth. It seems to be an East European thing - no offense to anyone here. The mother who has been plagued by severe cold sores all her life, expresses her "love" that way. I told her once how sad it was that parents give the herpex simplex virus to their children at all but she still believes that cold sores live dormantly in anyones body. Yeah, right.... Either way, her 3-year-old son developed almost permanent sores ALL OVER his upper lip so I assume that it is not true that you can only get infected once and then develop your own pattern of outbreaks. By the way, her son expresses his love for good buddies the same way. I had to prevent him from kissing my toddler and myself and eventually just avoided play dates. I know this sounds antisocial. I even made sure that there is NEVER any saliva transmission between my son and myself to avoid the spread of bacteria that causes tooth decay (strep mutans). Sorry, this is lengthy - but I just had to share my phobia...
