Primary PCI is treatment that occurs while the blood clot is still forming - usually within 12 hours of onset, but ideally within 2 hours of onset of symptoms. Rescue (not secondary) PCI is treatment that is given to try to get blood flow to heart muscle that has been starved of blood for some time, either because the patient delays seeking help or if the injected blood thinner 'thrombolysis' has failed. "Primary PCI," performed for acute coronary occlusion on an emergency basis, is also distinguished from "elective PCI," performed on a non-urgent basis because of risks that have been identified on angiography.
