Etg tests look for a specific metabolite byproduct of the consumption of ethanol. They are extremely sensitive and are good up to about 48 hours after consumption. Some studies say they are more accurate some say they are less accurate. ETOH is a test that looks to see if there is any ethanol actually present in the system at the time of analysis. These are cheaper and usually accurate only if alcohol has been ingested within a few hours. I am not an expert but have had a DUI and had to take both tests

Why would you have a positive ETG but negative ETOH screen?

Ethylglucuronide, ETG, is a direct metabolite of ethanol that forms in the liver only as a result of ethanol consumption. Therefore, even after ethanol, ETOH, has been completely removed from the bloodstream (by metabolism in the liver), there should still be ETG in the bloodstream. Thus, it is possible to have a positive ETG test even if an ETOH test is negative (indicating no ETOH left in the bloodstream).

What is an ETG test?

The ETG test is the Ethyl Glucuronide test. It is used to test for alcohol in urine. When ETG is found in the urine, it indicates that alcohol was consumed recently.
