You need to wait until you start to actually feel the withdrawal, otherwise it can possibly make you sick. So if you last took an opiate yesterday and your're starting to feel the "I wish I would just die" feeling, then it's time to take the sub.

Because of the nature of Buprenorphine, it is not a good idea to take it with other opiate pain medicine, because it blocks as well as stimulates different opiate receptors and can reduce their effects. Suboxone, one brand of Buprenorphine, is formulated with Naloxone (Narcan). It is a very powerful opiate "blocker" BUT in this case is only added to the formulation to prevent IV use of the drug, as the Naloxone is destroyed by stomach acids. Keeping all this in mind as soon as the opiate receptors are sufficiently clear of the Suboxone, other narcotic pain meds can be used to control your pain. How fast this happens depends on your Suboxone dosage as well how long you were on it, but after 48 hours you should start seeing an improvement in the effectiveness of your pain medication. This of course depends on what drug you are now prescribed, and if you were taking the Suboxone for pain control or opiate dependency maintenance. If you take any kind of opiate with suboxone the naltrexone Naloxone will send you in to VERY painful withdrawal. Do not do this, you might end up in the emergency room as well. If you are going to take opiates wait at least 3 days after you have taken suboxone if you want to actually feel the opiate because it takes that long for the suboxone to get out of your system.You will not get sick if you are already stabilized on suboxone. If you are taking suboxone daily and you take any opiate on top of it, it will not make you sick. The naltrexone Naloxone is only activated when the pill is crushed for potential IV use, otherwise not enough of the naltrexone Naloxone is activated to make you sick. I have taken pain killers within hours of taking my suboxone to try and relieve some of the pain that the suboxone was not taking care of and I didnt get sick.
