The smoke has to make it to your lungs to feel any effects, a very sensitive user MIGHT feel something from the smoke being in their mouths but it's probably because they actually inhaled it. To get high the smoke only needs to be in your lungs for seconds, but the longer it is held in the higher one will be. You just want the THC in the smoke to get into the blood, the longer you hold it in the more blood passes through your lungs where the smoke is. If it's good weed then you should be able to hold it in longer, bad weed should be held in as short as possible because it is mostly tar, basically just hold your breath for as long as you feel comfortable.
