When administering ceftriaxone to a patient with a known penicillin allergy, precautions should be taken to closely monitor the patient for any signs of an allergic reaction. It is important to have emergency medications and equipment on hand in case of a severe reaction. Additionally, it is recommended to consult with an allergist or immunologist to determine the safest course of action for the patient.

Penicillin is rarely used now a days. It is because it can cause anaphylactic reaction in few patients. Patient can die instantly out of this reaction. Even a test dose can lead to the death of the patient. Patient becomes unconscious in seconds. Patient gets severe spasm of the tracheobronchial tree and the blood pressure falls down. Pulse may not be felt at all. Nobody will like to take the risk of anaphylactic reaction of the penicillin. Never the less penicillin is still the life saving drug.
Tetracyclines (e.g. doxycycline), quinolones (e.g. ciprofloxacin), macrolides (e.g. clarithromycin), aminoglycosides (e.g. gentamicin) and glycopeptides (e.g. vancomycin) are all unrelated to penicillins and are safe to use in the penicillin allergic patient.
This will depend on the type of allergic reaction (mild rash vs. anaphylaxis) and the risk / benefit analysis of the prescriber. Generally, azithromycin (Zithromax) would be avoided in a patient with previous allergic reaction to erythromycin, but there may be rare circumstances where the potential benefit outweighs the potential risk if the previous reaction was mild, or if the patient can be monitored for a reaction and immediately given supportive treatment.
