# Flexeril (cyclobenzaprine) and traMADOL (Major Drug-Drug) SERIOUS PROBLEM: The risk of seizures may be increased during coadministration of tramadol with selective serotonin reuptake inhibitors (SSRI antidepressants or anorectics), monoamine oxidase inhibitors, neuroleptic agents, central nervous system stimulants, opioids, tricyclic antidepressants, other tricyclic compounds (e.g., cyclobenzaprine, phenothiazines), and/or any substance that can reduce the seizure threshold (e.g., carbapenems, cholinergic agents, fluoroquinolones, interferons, chloroquine, mefloquine, lindane, theophylline). These agents are often individually epileptogenic and may have additive effects when combined. Many of these agents also exhibit CNS- and/or respiratory-depressant effects, which may be enhanced during their concomitant use with tramadol.

POSSIBLE SOLUTION: Caution is advised if tramadol is administered with any substance that can reduce the seizure threshold, particularly in the elderly and in patients with epilepsy, a history of seizures, or other risk factors for seizures (e.g., head trauma, brain tumor, metabolic disorders, alcohol and drug withdrawal, CNS infections). ----

Yes. I was given both at the same time by my Doctor when I was having lower back spasms.

4-17-2012Cjonb and I have spent the last two days researching this. The results are as follows:This mixture may cause seizures...What apparently happened was that this problem was not noted in clinical trials of the drug, but was later reported in the field after the drug was approved. And so the labels were changed.A little background is necessary:* Tramadol itself can cause seizures, especially if the patient is overdosing* Trycyclic antidepressants are pretty extreme, as they have some nasty side effects. They're not quite the drug of last resort, but they're getting there.* If someone on tricyclics and tramadol seizes, it's very hard to identify the exact process that caused the seizure -- maybe the tramadol; maybe the psyche med; whoknows?* The Cyclobenzaprine molecule, like quite a few other compounds, has three rings in attached to a central structure, as do tricyclics. This alone would usually not be enough to counterindicate -- but in this oddball case, it is, or should have been.* These two meds were often prescribed together, and even today, probably still are.* The seizure risk, while extant, is not extreme.* Taking any drug means you're accepting a desired effect, and a few undesired ones. It's pretty much never purely all benefit. Tramadol is like this.

It may cause Serotonin Syndrome. Its unlikely, but some people are moresusceptibleto it than others.Serotonin syndrome is a potentially life-threatening condition associated with increased serotonergic activity in the central nervous system (CNS). It is seen with therapeutic medication use, inadvertent interactions between drugs, and intentional self-poisoning. Although classically described as the triad of mental status changes, autonomic hyperactivity, and neuromuscular abnormalities, serotonin syndrome is actually a spectrum of clinical findings ranging from benign to lethal.

Tramadol, which is the generic Ultram (a non-narcotic pain reliever), and Flexeril, which is a muscle relaxant can safely be taken together.
