it can be under the form of a crystal, which is very concentrated and most people don't ever see that one. it comes straight out of the lab.

it can be under the form of powder when the crystal is ground up, it is also really concentrated.

it can be under the forms of liquid drops, you will recognize it because it glows under a blacklight.

LSD, commonly known as "acid" can cone in various forms. I've seen it mostly on little square blotter paper, in small viles as a clear liquid, and as powdered pressed up pills. Sometimes it has cartoon logos on it when it comes on blotter paper. They look like small stamps almost.
