In doing this you increase the proportion of antibiotic resistant bacteria in the population. This ultimately stands to reduce the usefulness of antibiotics we have now in the future, and as there is not a great deal of money in development of new antibiotics, few pharmaceutical companies are inclined to do so.

There's another reason, especially in the case of elderly inpatients, if you give antibiotics that wipe out the normal gut flora you provide an opportunity for hospital transmitted infections such as clostridium difficile to set up home. This causes a vastly increased length of hospital stay for the patient and can put them at risk of a poorer overall outcome.

Also antibiotics are medications that can cause side effects like any others. Its unethical to subject patients to unnecessary side effects if antibiotics are unlikely to improve the situation (for example in a most likely viral ear infection, or in a sore throat where there is often little rationale behind prescribing the antibiotic).
