Among US astronauts the Challenger crew members Judith Resnik and Christa Mcauliffe both died- with all other hands- when their craft exploded barely 2 minutes into the trajectory. I do not know if any Female cosmonauts have died in space-related accidents . The other fatal spaceflight accident involving two females- also was the Space Shuttle Columbia- which occured on re-entry, not launch. In orbit proper- mercifully that hasn"t happened yet.
