He wasn't a monarch. He was a military governor, who styled himself 'Protector'. He was not popular with the public, who used to say "Lord protect us from Protectors".
History can not tell us this since cancer was not a known disease when he was alive. We can guess at any symptoms he may have had, but some symptoms are the result of many types of diseases.

A good example of what may have happened includes the British Republic, ruled by the Council of State, (1649-53) and then by the Protectorate of Oliver Cromwell (1653-58). Cromwell was able to seize control in the power vacuum left by the regicide. The republic-esque system was unstable, and when it fell to Cromwell's not-as-tyrannical son, it fell apart, and eventually was retaken by the monarchy. Of course, it is impossible to know for certain what would have happened, but a non-federalist system with a strong central government oft times leads to further consolidation, and abuse, of power.
