It depends. At the begging of the empire Rome was a republic headed by two annually elected consuls. The republic then fell and was replaced by absolute rule by emperors.

Emperor is a term which has been coined by historians. The Romans did not use it. They used the term princeps which is usually translated as first citizen or first among equals.
