That is a one sided opinion. It's an old joke that a fundamentalist is anyone who is more serious that I am.
In truth, a fundamentalist is someone who is serious about their belief of The Bible, and they actually follow as best they can, the tenants of Christianity.
It's someone who believes in the inerrancy of the Bible - that it is wholly and completely the word of God and true.

No, he is not the end result of such teaching for 2 reasons.(1) Was he really a christian at all? A fundamentalist christian would not break the 6th Commandment forbidding murder, let alone obeying Christ when He said not even to hate someone because that was the same as wanting them dead. Breivik was not obeying the tenets of being a christian, and just because he himself or others called him a christian findamentalist doesn't make him one.(2) This argument that:"Breivik killed 77 people. He is a fundamentalist christian. Therefore all fundamentalist christians are terrorists" is the same as saying:"Hitler killed millions. He was a Catholic.
