It is an individual that has attended Signalman (SM) A-school in the United States Navy and also achieved the rank of E-4 (Petty Officer Third Class). When the rating (job) and rank are combined, one would use the term Signalman Third Class (SM3).
As a side note, the rating of Signalman (SM) was merged into Quartermasters (QM) in 2003.

Not sure about male, but I found the first female Signalman was Elizabeth Nelson-Hein of Jasper, Alabama. The U.S. Navy disestablished the rating of Signalman in late 2003, reassigning visual communications duties to the Quartermaster rating. Signalmen were either absorbed into the Quartermaster rating, or allowed to switch to other job fields in the Navy. Many chose the Master-at-Arms rating, which expanded dramatically to meet the needs of the Navy in the War on Terror.

"HM" is the Navy rating for a Hospital Corpsman; the 3 means he/she is a 3rd Class Petty Officer.
In the modern US Navy, the equivalent rank to a Radarman Third Class is a Operations Specialist Third Class. They both serve as enlisted personnel responsible for operating and maintaining radar systems to provide situational awareness and tracking of objects in the Navy's operational environment.
