APT stands for Awareness Presentation Team. It is a group of cadets who go to middle and/or elementary schools and teach the kids about drugs, alcohol, and help with activities. I'm not aware of an APT ribbon, but i know there is a badge that can be worn on your uniform.

What is the difference between a medal and a award?

When you earn one, you're given the medal itself, a ribbon for your ribbon bar, a certificate that says what you did to earn the award and a set of orders for your military personnel file. Sometimes that certificate is pretty vague, especially if what you did was classified or they still haven't figured out what you actually did even though it was pretty neat.
