Chimneys were invented during the Middle Ages, during the 12th century.

Ancient Greeks and Romans did not have fireplaces, but only hearths, and the smoke rose through holes in the roof or at the tops of walls. The only heat was in rooms with high enough ceilings that they were unlikely to catch fire, and since that was the case, most houses had only one heated room, if any. Cooking was usually done out doors. The indoor fires made the air very smoky.

So you might say the first people ever to get cozy in front of a fire in a fireplace were medieval.
