There are many uninhabited small islands in Japan. The smallest of the 7 major islands is Shikoku at about 19,000 square kilometers.

One of the smallest inhabited natural islands is Mikimoto Pearl island, a tourist attraction in Toba (Honshu) at the western entrance of Ise Bay. It is only about 300 ft x 300 ft (8000 sq m) and about 150 feet offshore.
