The Black Death was an instance of what is called the bubonic plague that took place in Europe during the middle of the 14th century. Medieval people did not stop the Black Death from spreading. In order for the plague to continue, there had to be a fairly large number of carriers, mostly fleas and rats. Since the disease itself killed these carriers off, it ran out of them, and the disease stopped being epidemic.

It did not go away entirely. Just about every generation had one or more return visits of the plague. This continued until a cure for it was developed, in the early part of the 20th century. The later plagues were not as bad as the original Black Death, partly because people had a better control over rodent populations, but there may have been other reasons.

The Bubonic Plague, is still around, and reappears from time to time. Modern medicine makes it possible to cure it most of the time, if it is caught early enough. In addition, since the mechanisms spreading it are fairly well understood, it is easy to prevent it from spreading, so modern cases are usually isolated.
