Put up his forefinger.
the gladiators would be granted freedom if he was a slave
Shouting of course played a role, but it would be a thumbs up or the thumb pointing at the chest if that were the desired outcome. Death was not a regular outcome; gladiators were expensive and time consuming to develop and train.
Gladiators were mainly of two types - actual gladiators (a lot like modern boxers) or prisoners of war. Actual gladiators were fighters who wished to test their battle skills as training or sport in the battle arena (eg. Colosseum in Rome). These fought until they were tired or wounded or resigned. Killing each other was a very rare occasion and these gladiators were given money by the emperor for their skills. Prisoners of war caught by the Romans were a nuisance for them. To get rid of them, the Romans thought of a bright idea. They forced the prisoners to fight against their cellmates (even wild animals) with minimal weapons. A group of people left out were later executed. These people did not know how to fight (even though the Romans tried to train them to make it interesting). In the Roman Empire, Gladiators were mostly used for entertainment; for both the Emperor and the masses of citizens at the arena. Gladiators were basically slaves made to fight for the pleasure of the audience. :D hope I helped!
They did change. The first use of gladiators was at funerals, but it became so popular that it eventually became larger with a wider audience. Large arenas were built to hold games, there were training schools, and men who were taken alive in wars became gladiators. It was a big business for the wealthy of Rome to own or have an investment in a school. At it's peak gladiators had sea battles and fought wild animals as well as each other. Bets were made on who would win and some gladiators won fame and freedom ( though not many).
if they were good gladiators then they would just use fake teeth but if not most of there teeth would have rotted and fell out.
the gladiators would fight different kinds of Gladiators or other animals
Public entertainment was provided in huge venues, the most famous of which was the Colosseum in Rome. The entertainment took the form of combat, and people called gladiators fought each other, or wild animals like lions, to the death. Generally, gladiators were condemned criminals, prisoners of war, or slaves. Professional gladiators were free men who volunteered to participate in the games. Gladiators were paid each time they fought. Criminals who had been found guilty of murder and condemned to death went into combat without weapons. When one of the gladiators in a contest was wounded, the crowd went wild. If one of the gladiators felt he was defeated, he would raise his left hand with one finger extended. This was to ask for mercy. It is believed that the crowd voted for death or to spare him by signalling with their thumbs: thumbs up and the gladiator lived, thumbs down and his opponent was to kill him.
Literally, "blow of mercy" means a death blow to end the suffering of a wounded creature. It is used figuratively to mean an event which brings about the end of anything that has been failing.

Katniss had no personal connection to Thresh and it is not stated if Thresh's mercy was shown to the districts. It's likely that only Thresh and Katniss saw him giving mercy to her.

Yes the gladiators have won a few times. It was a fight between the gladiators was very hard and brutal some would have blood everywhere others would be dead while some would have a sword in their hand ready to fight again.
Yes they did. Gladiators were owned and trained by sponsors ( similar to football teams today) and the different sponsors would match gladiators against each other. Often betting took place on the outcome of the battle. This was a big business owning gladiators and training them. A lot of prestige came with having the best "line up" of gladiators.
