The title of imperator goes back to the earliest Roman Kings, who bore the title. The first of these was Romulus, so it would seem he had the title first; but of course, he was legendary, so who knows what they were doing.

The title of imperator was given to generals after impressive victories, and was needed for a general to have a triumph at Rome.
