The ultimate failure was, of course, the Second world War, which ended the Third Republic and produced the Vichy regime. After the war, the Fourth Republic provoded a new constitution which was designed to be better than the Third... but wasn't.
The Third Republic of France collapsed following the German invasion in 1940. Germany deposed the government and established a puppet regime under Marshal Petain in Vichy, France.
