Medicine has advanced basically because the changing times also means there are changes in research, more knowledge about a certain field in medicine or procedure in medicine will mean advancement and refinement of it.

But medicine has not really advanced gradually since prehistoric times. From the earliest times until the late 19th/early 20th century, medicine advanced very little and even the most expensive and famous doctors simply did not know what caused most diseases nor what could effectively cure them.

It was only with the discovery in the late 19th century of the link between poor hygiene and infections and Infectious Diseases, of x-rays and anesthetics that started to make careful surgery possible and of pennicillin in the 20th century (to mention just a few) that medicin began making big advancements. Today it is the enormous amount of research and development by universities and pharmaceutical companies that keep the pace of development going.
