The Roman empresses originally had no title. Livia, the widow of Augustus was granted the title "Augusta" and that's as far as it went. There may have been some women who used the title in the later part of the empire, but it was not "official", that is, granted by the senate.
Octavian was adopted by Julius Caesar as his son. After Caesar, the civil war broke out again and destroy what left the Roman Republic. Octavian and Caesar's grandnephew were joined with the General Antony and Lepidus. In 43 BC, they took control of Rome and ruled for 10 years as the second Triumvirate. then later, Octavian forced Lepidus to retire. he and Antony became rivals. soon, Antony met Queen Cleopatra of Egypt. Antony went with Cleopatra to Egypt. Later Antony and Cleopatra committed suicide. Octavian restored some aspect of the republic. he became the unchallenged ruler of Rome. Eventually he accepted the title of Augustus.
He prefered Princeps.
He was crowned Emperor by the Pope in 800, but didn't use the title. Note that the notion that he was crowned Holy Roman Emperor is a later fiction.
The first was the Empire's founder, Charlemage. He was "Holy" because his rule had been blessed by the Pope - Leo III crowned Charlemagne personally. He was "Roman" because the Holy Roman Empire was meant to return to Europe the order and cultural life that had been lost when the original Ancient Rome fell.
