1) buy index shifter 2) buy new shifter wire 3) remove old shifter wire 4) remove old shifter(may require grip removal) 5) install friction shifter(if needed, install grip) 6) run new shifter cable 7) clamp cable at derailer as the derailer will have gone slack when you removed the old cable, make sure the shifter is in the right position to pull cable when you clamp the new cable in place. Trim & crimp excess cable, go ride your bike. Note:if your bike have integrated brake and shift levers you either have the choice of getting new brake levers as well, or continue to use the old but with the shifters disconnected.
