A lock washer overlaps itself when it is new so that when you tighten the nut down the pressure causes it to flatten out and in turn put pressure against the nut and whatever it is against. The pressure plus the gap in the washer helps prevent the nut from turning.

What are the isolation and lock off procedures or permit to work procedures that apply to the equipment being maintained?

Isolation and lock-off procedures involve shutting down equipment, disconnecting energy sources (such as electrical, hydraulic, or pneumatic), and securing them to prevent accidental re-energization. A Permit to Work (PTW) system ensures that all hazards are assessed, and necessary safety measures are implemented before maintenance begins. This includes clearly labeling isolation points, using lockout/tagout devices, and obtaining authorization from a responsible supervisor. Documentation of the procedures and verification that the equipment is de-energized are critical steps to ensure worker safety during maintenance activities.

What is the definition of tagout?

the placement of both a lock and tag by an authorized worker on an energy isolating device in accordance with AFOSH 91-502 Chapter 21 to ensure that the energy-isolating device and the equipment being controlled cannot be operated until the lock is remove
