The lid will often have 2-3 numbers inside it. Note them all and go to a major plumbing supplier, they will be able to track the number and order a lid.
At least the lid should be replaced. If just the lid can not be found, buy the minimal parts that include the lid. Replace any parts included just in case. If the water keeps trickling out and does not build up enough in the tank, it will not flush.
