The HIB number, or Health Insurance Benefits number, is typically located on the front of your Blue Cross Blue Shield card. It may be labeled as "Member ID," "Policy Number," or "Subscriber ID," depending on the specific card design. Look for a series of numbers and letters that identify you as a member of the plan. If you have trouble finding it, you can also refer to the information provided in your member welcome materials or contact customer service for assistance.
It depends on the plan you are on. Call the benefits office number on the back of your insurance card and ask.
The coverage will depend on your plan -- what your employer pays for -- and not on the insurance company. Contact the number on the back of your insurance card to find out what's covered.
Call them and ask for another one. They will mail it out to you in a few days. You should have some record of the phone number on a statement of benefits or through your employer. Note that you doctor usually has a copy of the card on file that will have the phone number and even account number.
Normally you will only have coverage in an emergency situation, and then it is on a reimbursement basis. You would pay for the emergency service and file for reimbursement. Call your member services phone number on your BlueCross BlueShield card for details.
Your best bet would be to call the helpful representative number listed on the back of your card, after first checking with your employer HR benefits department.
Empire BCBS covers abortions for insureds whose policy includes that service. Your employer may or may not have chosen to include that coverage. For information specific to your insurance contract, call the phone number on the back of your card and give your group number.
According to a "Question and Answers" blog at the Blue Shield website, Blue Shield should cover up to 80% of the retail cost of the shingles vaccine. That would make it around $40.00. However, I would bet that it depends on the level of Blue Shield coverage you have and if you have Medicare Part D coverage.
The card number for "Blue-Eyes Toon Dragon" is 53183600.
Blue Cross and Blue Shield of Louisiana offers different types of plans - including an HMO. For a list of providers in Louisiana search the BCBSLA website at http://www.bcbsla.com/web/reddotcm/html/719_provider_directory.asp For IN network providers (doctors and facilities) in any other state in the U.S. search here: http://provider.bcbs.com/ A provider's status can change, always call and ask if they are a part of the "Blue Card network" before services are rendered. Good luck and good health.
The answer will depend on whether the nurse practitioner's practice has a contract with BCBS of Mississippi, and whether your particular BCBS plan requires referrals, prior authorization for out of network providers, etc. Contact your health care plan using the number on the back of the card to ask about the particular practice of interest.
It isn't up to the Carrier, for example Blue Cross or Aetna to cover it unless you are on an Individual Policy. You have to check with your Employer or Group plan to see if it is a covered benefit. While there has been recent legislation governing the parity of counseling benefits, it stopped short of requiring employers to cover it. So it is up to each employer to decide if they want to offer those benefits or not. Try calling the Customer Service number on the back of your ID card they will be able to tell you if your plan covers that or not and how it is covered. Good Luck!
Blue Cross Blue Shield has many different plans. These plans each have different benefits. What is covered under one BCBS plan may/may not be covered under another. It all depends on how the plan was built by whomever your insurance is provided through. Many BCBS plans do cover the cost of a CPAP machine based on medical necessity. I would contact your BCBS plan using the member benefits number found on the card and ask them if the cost of a CPAP machine is covered under "YOUR" benefits.
