Did Germany declare war over the formation of the league of nations?

No. Germany did not declare war. After world war one, Germany's fascist leader Adolf Hitler slowly began to violate the Versailles treaty Germany had signed when they were defeated. The league of nations failed to properly deal with these issues and Germany eventually began to build a powerful military. Hitler first began his campaign by acquiring German speaking territories, but the league of nations kept silent until Germany invaded Poland on September 1 1939. Britain and France declared war on Germany on September 3 1939.

What did Wilson insist be part of the Treaty of Versailles in order to prevent future wars?

He wanted the League of Nations to be set up to prevent another World War. The League was started, but, even though the USA did not officially get into it, it failed when the Japanese invaded China, and the League of Nations could not do anything about it. Then Italy attacked Ethiopia, and Germany kept making more and more demand upon the other nations until it invaded Poland on September 1 1939 and WWII started. The League of Nations without any power to back it up was just a lot of talk that Japan, Italy, and Germany paid no attention to.

Was Germany broken into smaller nations?

Until Otto von Bismarck unified Germany in the 1860s creating the "Second Reich" Germany consisted of many small independently ruled nations. From 1920 until 1939 the East Prussian state of Germany was separated from the rest of Germany by the Danzig corridor of Poland, but was still politically part of Germany and not a separate nation. From 1945 until 1990 Germany was partitioned into West Germany and East Germany.
