1. establish what you are asking and in what sequential order.
2. Create a paperbased (or word based) questionaire making sure that each question has its own number and that potential answers are also sub categorised i.e.

1.2 Male
1.3 Prefer not to say
Then develop a database which allows for the questions to be added in the same sequential order - There are ready made models in the freeware environment that could help.
Set up queries to bring all the questions together and account for each of the options given. Try wherever possible NOT to ask for suggestions or direct in put as this causes huge amounts of consolidation.
Workout how/where you will be issuing the questionnaire - is it paper based - or word based (electronic) or will you use an on line facility to allow for direct input. The later has some very good available applications out there that could assist in data collection
