If you're a British seafarer, or are working on a UK-registered ship or yacht, you're required to hold a seaman's discharge book. You'll also need a British seaman's card if you're a British citizen. It costs £40 to apply for a British seaman's card and £40 to apply for a Seaman's discharge book.

If you are renewing, you may also require STCW 95 Certificate of Competency.

Certificates of Competency are issued every five years on proof of training or sufficient sea time service and a valid medical issued by an approved MCA doctor. Revalidation is £36.00, plus £10.00 for overseas postage. (If the postal service in the receiving country is not reliable it may be better to use a courier service, which costs an additional £30.00).
