Actually, nothing. There is no position in the US Government known as "First Lady". She is the wife of the President- but being a relative does NOT make her officially in charge of anything. It is a position of honor, but not an elected office.

Nothing, unless the President appoints her to work or lead certain committees or initiatives. For the first period of time, First Ladies only served as the hostess at official state functions.
