CDL regulations are managed by the federal government rather than the state. The only case where they are managed by the state is for intrastate driving, which means not only do you have to drive only within the state, but the load and truck must not be interstate either. So the vast majority of the time, a CDL driver is dealing with the federal regulations unless he truly drives locally.
The DOT used to forbid anyone using insulin whatsoever to drive without an exemption, and required 3 years of experience driving on insulin in order to get the exemption. Due to the paucity of intrastate jobs, this was a near catch-22 situation.
However, recently the regulations have changed. The requirement for an exemption now includes a statement from a regular Doctor Who does DOT certifications, a statement from an endocrinologist and a statement from an opthamologist or optometrist.
Note that the endocrinologist statement says that the endo is familiar with your case and knows you know how to manage your sugar, that you are educated on this topic at least annually, and that your regular bg readings are OK (not just an A1c, but you need a log of regular bg readings for the endo to evaluate). So in reality, this exemption is only applicable to well-controlled, educated insulin-users.
Non-insulin using T2s do not need to apply; they have always been permitted to drive.
