It depends on the type of account. For a bank account, you appear at the bank with suitable ID and funds to deposit, and fill out the documents they give you.
If you have an adult to sign with you, any kind you want. You have to be 18 to open an account without an adult. As long as you have some money, because usually (although not always) you have to keep a small minimum balance in a checking account. But you can buy any kind of service a bank offers as far as I know.
No. A person can only open an account for himself. They cannot open an account on behalf of someone else. It is illegal and it should not be done. If I want a bank account, only I can open it and cannot ask or expect anyone else to do it for me.
If you are a minor (less than 18 years of age) then yes you need the signature of an adult (Preferably your parents) in order to open a bank account. If you are an adult (more than 18 years of age) then you are free to open an account by yourself and it is not mandatory to have an adult supervise you.
Well you can open an account when you are 12 or older. But I am not sure if it supposed to be parent only
If you want to open an individual account then you must be atleast 18 years old. But, if you are a minor (less than 18 yrs) you can always open a joint account with any of your adult family members like mother, father, elder brother etc. banks do not say no to any customer who wishes to open a bank account

I don't think it is legal for a 14 year old to do anything like that. You need to get an adult (preferably a parent) to open an account and you can have an agreement that it is your money. Make sure it is someone you trust.
Yes, definitely. But, the fifteen year old would need his parents or legal guardian to be a joint applicant and holder of the account. Banks will have no problems giving you an account as long as an adult is a co-holder of the account.
