Complete and submit a New York application for reciprocity. Under the Certification Route, choose Route 2 box stating Reciprocity/CNA From Another State. Include a copy of your current nursing assistant certification, certification number, and date of expiration. Your certification must be current and in good standing in order to be accepted. Include the $50 reciprocity fee and mail application and documentation to Prometric- Attn: NY Nurse Aide Program- 7941 Corporate Drive, Nottingham, MD 21236.
