It depends entirely on the level of disability. If you have 100% disability, you will receive 100% of the highest level of pay you received, i.e. if you reached the rank of Staf Sgt. E6 with fifteen years time in service and two years time in grade, you would receive 100% of the pay commensurate with that rank and grade.

Yes, and it's recommended to add supplemental disability insurance to cover closer to 100% of your income. If you have disability insurance through your employer, your benefit will be capped at 66% of your income. The benefit received from a group disability policy (through your employer) is taxable. Supplemental disability insurance benefits are not taxable. You can add Catastrophic rider on an individual disability insurance policy, to cover up to 100% of your income in combination with your existing employer group DI.

No. I was recently laid off while on disability. The law in Texas says you have to be able to physically be able to work so if you are on disability you can't work. This is NOT correct! Social Security Disability allows you to work even if you are 100% disabled! They limit how much you can earn. (Your unemployment income will NOT reduce your disability payment because SS does not consider it to be employment income!)
