yes or no
ADA requires employers to REASONABLY accommodate ONLY "qualified individuals with a disability" - workers who can still perform all work functions the employer calls essential. If you can't perform ALL essential functions, no accommodation required. If you do not request accommodation by suggesting one, no employer duty to initiate reasonable accommodation. If there is an accommodation that would let you perform all essential job functions, the employer makes a unilateral decisioin whether it is reasonable - inexpensive, does not violate a contract, or unfairly burden other employees.
No. Employers may only ask employees if they are able to perform the essential functions of the job, with or without reasonable accommodation. Any questions about pregnancy are against equal employment and ADA laws.
If they don't perform given proper, reasonable accommodation, then yes.
Just because it's in the form of a question doesn't mean you should ask it here. If you can't figure out what the answer to that is, you don't deserve to be hired. On the off chance you don't understand the words: they're asking "Can you do the job without us buying expensive special equipment? If you can't do it by yourself, what would you need in order to be able to do it?" So, if you want to be a librarian, and the job requires you to re-shelve books on a shelf that's 8' high and you're 4'6", the "accomodation" you need is a short ladder to stand on.

Worklife was about the same as now. Profoundly disabled workers were not accommodated if that would be too expensive. Workers who could not perform every job duty the employer calls essential had no protection - like today. Workers whose condition was not permanent and sever had no employment rights - like today. Before ADA, employees had no mechanism to TRY and arrange reasonable accommodation. Qualified individuals with disabilities who were capable of performing all essentila job functions had no protection. That cahnged.
