A "Will" would serve your purpose. If you died without a will you would be "intestate" and the state would step in to 'fairly and equitably' handle things, including the disposition of your body. If I were you I would contact the medical school or hospital to whom you wish to donate your body, and ask them for assistance in this matter. I am sure they would have some experience in this area.
