Although it sounds simplistic, the basics of an initial police report should contain the following basic elements:

WHO is the victim or complainaint?

And - if the victim believes they know the perepetrator(s),

WHO who does the victim believe committed the crime.

WHAT was it that happened?

WHEN did it occur?

WHERE did it occur?

HOW did it occur?

The information gathered via the initial police report, usually taken by the first responding patrol officer, is perhaps the most important record that will impact upon the case since it 'frames' and identifies the offense which was perpetrated.

Any investigative reports that follow, will simply build upon the basic framework of the intiial report.
