These are two adjectives meaning - next or following.

Prochain is generally used when the context is the present while "suivant" is used in relation to the temporal definition given by the context.

It is the same difference between "tomorrow" and "the day after" in English

examples

Je serais là la semaine prochaine. I'll be there next week.

Je ne pourrais pas venir la semaine prochaine, mais je viendrais la semaine suivante.

I won't come next week, but I'll come the week after.
