One typically asks a person's name in Chinese with 你叫什么(名字) - ni3jiao4shen2me5 (ming2zi4) - literally "you called what (name)". This however is on the rude side, and should not be used towards any sort of superior or authority figure. A more polite phrase is 您贵姓 - nin2gui4xing4 - literally "your honorable surname". The person will then reply with their last name or family name, which in Chinese is said first. They may also inclued their first name, but not always.
