You may know all of the words but if you have an accent then they may not sound right. Edit: it is impossible for one person to know all the words of any language, but I know a lot of people whose first language is spanish and are fluent in english, my first language is English and I'm fluent in Spanish.

The only way a person can learn fluent from birth is been around a lot of different languages. It is not easy learning different languages.
Why you need English and to be fluent in English, because wherever you go and whatever you do you can communicate to each other using the English language. The English language is spoken in so many parts of the world that even a person leading a simple life may, at least need to or wish to, understand what is being said. Communication by language is the key to human interaction.
