I have been asked to do one of these in the past for a rescue. I followed a form that asked questions such as "how long will the dog be alone each day" and "how often will the dog be exercised" etc. I was not asked to do any follow up once the dog was placed. I'm sure resources are limited and for any rescue to follow up on all of the dogs they place would be overwhelming.
