Yes, cats tend to leave home to go die alone, when they know that they are too old or too sick to live on. They know when the time is, it's animal instinct.
Often it's not very far away from their home, though.
Unless they get killed by accident (roadkill, etc.), then it is - of course - not something they decide for themselves.
