Sampson, a Shire horse gelding foaled in 1846 in Toddington Mills, Bedfordshire, England, is the tallest horse ever recorded.

Sampson, owned by Mr Thomas Cleaver, stood 21.2½ hands high (i.e. 7ft 2½in or approx 2.2m at his withers) by the time he was a four year old, when he was re-named Mammoth.

His peak weight was estimated at 3,360 lb (1,520 kg).

In the guinness book of records is the horse Radar (still alive) the largest horse on earth. He is 2.10 meters high and is a Belgian (what is weird because the maximum height of Belgians is 1.70 meters)

EDIT: As of 2013, the tallest living horse according to the Guinness Book of Records is a Belgian by the name of Big Jake. He is 11 years old and stands 20 hands 2.75 inches (210.2 cm) at the withers.
